Short Breaks are available for parents and carers of children with disabilities. To qualify you must care for a child who:

  • is aged between 0 and 18, or 19 to 25 and have a personal budget
  • has a diagnosis of special educational needs and/or a disability
  • lives within the administrative boundaries of Essex County Council (excluding Southend and Thurrock)

Before you begin, you'll need:

  • a scanned confirmation of the diagnosis for your child/young person - we will ask you to attach the file to this online form

If you do not have a scan of the conformation or are unable to attach a file, please email shortbreaks@essex.gov.uk.

Once we receive your details, we'll be in touch to confirm your registration.

You can only register one child per form. If you have another child with a disability, please complete a new form.

 

What happens to my information?

We’re collecting your personal information to process your request to register for Short Breaks. We will not share it with any other person or organisation unless the law says we must. Please note your registration will not be complete until we receive your confirmation of diagnosis. As DLA does not confirm diagnosis we are unable to accept this as proof of diagnosis. If you are not able to attach to this form please email to shortbreaks@essex.gov.uk quoting the reference number you will be given when you submit this form.
