Before you submit your entry, please ensure that you have carefully read the Eligibility Criteria (Who can Enter?), the Terms and Conditions and the guidance with each question in the Challenge Prize entry form.

At this first stage we want you to demonstrate creativity and innovation through the application process, sharing with us the idea or concept that sits at the heart of your proposal. The entry form is designed to provide us with an overview of your idea and allow you to demonstrate your potential. The limited word count reflects the level of detail we are looking for at this first stage.

Please respond directly to the questions, stick to the word limits and use anonymised examples or evidence to back up your statements and proposal. We may call you to clarify or find out more about your idea, so please ensure you include a phone number and email address on your entry form.

If you have any further questions relating to the Challenge Prize that have not been answered by the Terms and Conditions of the prize, please email Guidance on what the judges will be looking for is outlined at the start of each section of the application form.

All applications must be submitted by 12 (Noon) on 13th April 2018.

We are collecting your personal information in order to process your application for the Challenge Dementia Prize. If you are shortlisted as a finalist we will share your application form with our support network in order for them to determine how best they can support you with the development of your idea. No personal contact details will be shared without further discussion (or) without contacting you further.
