  • I am blind
  • I receive the Higher Rate Mobility Component of the Disability Living Allowance
  • I receive a Personal Independence Payment (Mobility Component) - Where my ‘Moving Around’ descriptor indicates that I cannot walk more than 50 metres
  • I receive a War Pensioners' Mobility Supplement
  • I have received a lump sum benefit under the Armed Forces and Reserve Forces Compensation Scheme within tariff levels 1-8 (inclusive) and have been certified by Veteran’s UK as having a permanent and substantial disability which causes inability to walk or very considerable difficulty walking

  • I have a permanent and substantial disability which means that I am unable to walk or that I have very considerable difficulty in walking.
  • I drive a vehicle regularly, and have a severe disability in both arms that means I am unable to operate all or some types of parking meters.
  • I am applying on behalf of a child less than 3 years of age who has a condition requiring transportation of bulky medical equipment, or has a highly unstable condition which needs quick access to transport to hospital or home, or where there may be a need to stop to deal with an urgent medical problem
  • I qualify under Special Rules - we have special rules for people who are terminally ill (this means people who have a progressive disease and are not expected to live longer than another 12 months). Your application will need to be supported by a specialist nurse or consultant.
