Before you begin

When to use this form

Read the Essex Coroner’s Service information on Making a Coroner referral.

Who can use this form?

  • General physicians
  • Hospice doctors

This Referral Form has been amended following feedback received from service users.

Information and documents needed to complete your referral

  • your General Medical Council (GMC) number to hand
  • all of the deceased’s details including copies of:
    • the death certificate, if available
    • supporting documents such as additional medical notes eg recent operations, procedures or hospital admission / discharge notes must be uploaded with your referral

How long will it take?

You will need between 10 and 15 minutes to complete. You can also save a partially completed form. A link will be sent to your email address and you can use this to go back to your saved form.

What happens next?

A summary of the completed form will be sent to the email address you provide in a confirmation email after submission. Our Coroner’s team will be in contact within 5 working days to advise you of the next steps.
